A C Tech Distinguished Alumni Award Nomination Form
The A C Tech Distinguished Alumni Awards Program was created by the A C Tech Alumni Association as part of the celebrations of 75th Jubilee of A C Tech to recognise highly distinguished Alumni for extraordinary achievements and contributions to their professions, communities and academic fields. An Alumna/Alumnus could nominate an outstanding individual by using this online form. The last date for Distinguished Alumni Award nomination is extended to October 25, 2019.
GUIDELINES:
1. Please fill in this form to nominate for the award.
2. Any A C Tech Alumna/Alumnus are authorised to nominate any alumnus/alumna from any batch.
3. Self nomination is not allowed.
4. Download the Digital Nomination Form from “www.actechalumni.org”. Fill up all the details in the Digital Form, save as PDF or JPEG and send it as email to “awardsactech@gmail.com”.
5. The decision of the Jury Panel is final and no correspondence or query would be entertained.
6. The last date for submitting Nomination Form is extended to 25th October 2019. The Jury Panel is scheduled to select the Distinguished Alumni by last week of October 2019. A very learned jury is already in place in this regard.
7. Nominations received are kept valid for 3 years. I.e. nominations not selected this year will be considered for next two years.
